Procedures for Spring 2011

Future Stars and U10-U14 Age Groups

Please download the appropriate Registration Form located in the download area on the right side of this page.

Coaches need to make a copy of the registration and birth certificate for each player, then submit the original to the Registrar. Each registration form must contain the coach’s name, as well as all other required information. Players new to Select, and those who have not been active for two years must submit a copy of birth certificate. Please mark new players on registration form.  Note that after the birth date is verified and entered into the Indiana Youth Soccer database, the birth certificate copies are shreaded for your security.

A team roster sheet must accompany the registrations.  Any player not on the roster sheet is ineligible for Select practices or play.  Playing any unregistered player will result in the forfeiture of the game.

Each registration must be accompanied by a check for $65.00 for the current season, or $120 for the entire year, made out to Select Soccer. It is helpful if the player’s name is on the check. Do not staple the check to the registration form; a paper clip is preferred. It is extremely helpful if the coach collects the registrations and puts them in alphabetical order before submitting.

If a uniform is needed, the player needs to obtain and pay for this uniform at Soccer Plus.

Team registrations are due by March 19 for the Spring 2011 season.  An additional week may be available, until March 26.  Please contact the Registrar to request the extension.  Registrations received after this date will be charged a $5 late fee per player.   As soon as you have the minimum number of players for your roster (7 for small sided, 10 for full sided) please submit immediately, so we can register the team.  No player will be allowed to practice or play without a registration form being completed and the fee paid.

Future Stars

Registrations will be accepted until April 15.  Print the Future Stars registration found in the links on the right.  Complete the registration and send it, the $25 fee and a copy of the birth certificate (if new to our program) to:
